Effective in 2013, the Centers for Medicare & Medicaid Services (CMS), mandated by the Patient Protection and Affordable Care Act, established the Hospital Readmissions Reduction Program (HRRP), which penalizes hospitals for “excess” readmissions for certain disease categories. Of the 15 million adults who have been diagnosed with COPD, 40 percent of patients with exacerbations fail to be treated with the commended therapies, and nearly half receive one or more therapies that are inappropriate or even harmful to them. As a result of these failures in treatment and follow-up care, rehospitalizations are likely to occur. By adhering to care standards from the outset, along with relevant post-acute interventions, 30-day readmissions can be reduced.
