New Jersey Selects Harmony Information Systems for Long-Term Care Initiatives

Division of Aging and Community Services implements Harmony for Aging Services to Achieve Rebalancing Goals

RESTON, VA – August 18, 2009 – Harmony Information Systems, Inc. (Harmony), the leader in integrated health and human services management, announced today that the state of New Jersey has selected Harmony for Aging™ to achieve its goals of statewide rebalancing of long-term care.   Harmony will be deployed to more than 2,000 users across the state, including all levels of long-term care, to support the objective of establishing a more equitable balance between the proportion of Medicaid expenditures used for institutional services and those used for community-based programs.
Led by the New Jersey Division of Aging and Community Services (DACS), this project will achieve important goals mandated by the Independence, Dignity and Choice in Long-Term Care Act signed into law by Governor Jon S. Corzine in 2006. Harmony for Aging will be used to create single points-of-entry for long-term care and will integrate across stakeholders, including workers at the state, counties and home and community-based services providers.
Harmony will serve as the unifying information technology system in achieving several key DACS goals for long-term care rebalancing in New Jersey:

  • Statewide roll-out of Aging and Disability Resource Center model
  • Clinical Eligibility Determination
  • Nursing Home Diversion
  • Fast-tracking financial approvals
  • Service Coordination
  • Quality Assurance (CMS Quality Framework)
  • Transfer of long-term institutional funding to Home and Community-based Services Waiver programs
  • Consolidation of three Medicaid Waivers into one program – the New Jersey Global Options Program
