Collaborative, SaaS-based CCTP Solution Will Improve Success of Care Transition Intervention for High-Risk Medicare Consumers in Virginia
RESTON, VA – November 14, 2013 – Mediware Information Systems, Inc. (Harmony), the leader in integrated health and human services management, announced today that the Eastern Virginia Care Transitions Partnership, a formal coalition spearheaded by Bay Aging, has selected the Harmony for Care Transitions solution to operationalize the Community-based Care Transitions Program (CCTP) grant and deliver enhanced care transition intervention to over 11,000 high risk Medicare beneficiaries spanning 17 counties in both rural and urban areas.

The CCTP was launched in 2011 as part of the Affordable Care Act to improve transitions of Medicare beneficiaries from inpatient hospital setting to other care settings, reduce readmissions for high risk beneficiaries, and document measurable savings to the Medicare program. Under this program, 102 community-based organizations (CBOs) will use care transition services to effectively manage Medicare patients’ transitions and improve their quality of care.
Harmony for Care Transitions is a purpose-built solution that enables CBOs to plan, track, manage and report on the services they provide and to provide a comprehensive view of consumers across all programs that serve them. Using Harmony for Care Transitions, CBOs can easily identify and track all care transitions episodes separate from other programs within one single database, eliminating siloes, using processes and workflows consistent with other programs. Harmony for Care Transitions tracks all required follow ups in a single database and provides powerful assessment tools to track how a consumer’s condition has changed during the episode and ensure that proper care is provided immediately to prevent hospital readmission, ultimately resulting in successful transition and maximized reimbursement.
The Eastern Virginia Care Transitions Partnership is a formal coalition led by Bay Aging and consisting of three health systems, seven acute care hospitals, five Area Agencies on Aging, and a multitude of other healthcare providers.
