Long-term Care Professionals to Gather in Cincinnati to Discuss Ways Technology Will Help Lead Change as More Consumers Receive Services Funded by Medicaid and Medicare

Dr. Coleman will also offer his view on the role that family caregivers should play in care transitions episodes, the challenges CBOs may face in implementing care transitions, and how organizations can best optimize resources to deliver on this model.
