Georgia Selects Harmony Information Systems for Money-Follows-the-Person Programs

Proven, On-Demand Solution Will Automate Manual Processes

(Harmony), the leader in integrated health and human services management, announced today that Georgia has selected Harmony’s Money Follows the Person (MFP) solution to streamline and automate manual processes for the state’s Money Follows the Person program.

The MFP Rebalancing Demonstration Program is part of a comprehensive, coordinated strategy to assist states in their efforts to rebalance long-term care services, decreasing their reliance on institutional care and creating community-based long-term care opportunities to enable the elderly and people with disabilities to fully participate in their communities.  MFP provides transitional services to Medicaid-funded, institutionalized individuals who choose to live at home in their community rather than remain in a nursing home.  To date, 43 states and the District of Colombia are participating MFP programs.

Harmony’s MFP solution enables states to rapidly realize the goals of their MFP programs by automating virtually all elements of long-term supports and services programs.  Harmony provides essential support at each stage in a typical MFP operational process, enabling organizations to efficiently implement MFP and to provide Medicaid beneficiaries living in nursing homes or other institutions the opportunity to live in their homes and communities.  Harmony’s solution provides web-based access by all referral sources for the MFP program, such as Aging and Disability Resource Centers (ADRCs), Area Agencies on Aging (AAAs), nursing home diversion programs, outreach, and other referral sources.  With built-in business intelligence reporting, Harmony’s MFP solution helps agencies track essential data for MFP grant reporting and for planning and program management purposes.
