Capital Area Agency on Aging Selects Mediware Information Systems for Care Transitions

Modern, Collaborative CCTP Solution Will Help Reduce Hospital Readmissions for High-Risk Medicare Beneficiaries in Louisiana

RESTON, VA –  February 27, 2014 – Mediware Information Systems, Inc. (Harmony), the leader in integrated health and human services management, announced today that the Capital Area Agency on Aging in Louisiana has selected the Harmony for Care Transitions solution to operationalize the Community-based Care Transitions Program (CCTP) grant to increase the number of successful care transitions for high-risk Medicare beneficiaries.  This announcement follows a number of key care transitions wins including Eastern Virginia Care Transitions Partnership, Maui County Office on Aging, and the Sonoma County Area Agency on Aging.

The CCTP was launched in 2011 as part of the Affordable Care Act to improve transitions of Medicare beneficiaries from inpatient hospital setting to other care settings, reduce readmissions for high risk beneficiaries, and document measurable savings to the Medicare program.  Under this program, 102 community-based organizations (CBOs) will use care transition services to effectively manage Medicare patients’ transitions and improve their quality of care.
