Flexible, Purpose-Built Solution Will Help Increase Successful Transitions and Maximize Medicare Reimbursement
RESTON, VA – November 19, 2013 – Mediware Information Systems, Inc. (Harmony), the leader in integrated health and human services management, announced today that the Sonoma County Area Agency on Aging in California has selected the Harmony for Care Transitions solution to operationalize the Community-based Care Transitions Program (CCTP) grant and the number of successful care transitions for high-risk Medicare beneficiaries.
The CCTP was launched in 2011 as part of the Affordable Care Act to improve transitions of Medicare beneficiaries from inpatient hospital setting to other care settings, reduce readmissions for high risk beneficiaries, and document measurable savings to the Medicare program. Under this program, 102 community-based organizations (CBOs) will use care transition services to effectively manage Medicare patients’ transitions and improve their quality of care.
